What does it mean when you cannot scream in a dream?

One of the most universally recognised dream experiences. Dreamers describe the effort as physical and the silence as total.

In sleep paralysis this has a direct physiological explanation. The muscles genuinely are not responding. In an ordinary dream it is more likely the same mechanism that produces slow-motion running: motor commands issued during REM produce no feedback, and the dream constructs the experience of effort without result.
