What does it mean to dream you could not save someone?

The dominant pattern and one of the most distressing dreams people report: running towards somebody, reaching, and never closing the distance.

The failure is mechanical, not moral. That is the useful sentence here. Effort producing no effect is a documented feature of dreaming and it runs through everything: legs that will not run, hands that will not grip, a voice with no sound. A rescue dream is that same failure with the highest possible stakes attached.

Dreamers do not conclude that. They conclude they were too slow, or not enough, and carry it into the morning about somebody who was never in danger.

Where the person is a child and the dreamer is their parent, these reports are close to universal and are among the most commonly described parental dreams there are.

Where this reads well

Recognising it as the same failure as the running dreams removes most of the guilt, and dreamers usually confirm the pattern in themselves straight away.

Where it is harder

The guilt is the harm, it is entirely undeserved, and dreamers often carry it for days.

What stopped me, exactly? Did my body work? Do my legs fail like that in other dreams?
