Dreaming you cannot explain your symptoms
What does it mean to dream you cannot tell the doctor what is wrong?
Examination reports are physical, and dreamers frequently mention being unable to explain what is wrong with them.
That connects this to the speech-failure material rather than to anything medical. The words do not come, or come out as something else, or the dreamer knows exactly what to say and cannot deliver it.
It is the same mechanism as the interview that goes blank and the courtroom defence that will not arrive: during dreaming sleep the machinery of speech is switched off, and the intention returns nothing.
A distinct group follows real illness, the dreamer's own or somebody else's. Those reports are continuous with the waking situation and should be read that way rather than symbolically.

Did I know what I wanted to say? Did any sound come out? Is somebody actually unwell?
