What does it mean to feel followed in a dream without seeing anyone?

Certainty without evidence. You know something is behind you and you never turn, or you turn and there is nothing, and the certainty survives anyway.

The sense of presence is a recognised feature of the sleep-wake boundary and appears constantly in sleep-paralysis reports, where it arrives before anything is seen. It appears to be generated directly rather than inferred from something noticed, which is why it feels like knowledge rather than suspicion.

That is one of the more genuinely strange facts about human sleep, and it is well documented.
